As part of the admissions procedure, many schools require students to submit to a school physical. The school physical report is a document for the school to obtain a general picture of the student's health, immunizations, medical history and, if necessary, any major health problems or special needs.
We suggest doing the following to help you and your child prepare for their physical exam before returning to school:
Talk to your child about what will happen during the physical exam to help him feel comfortable.
Write a list of family medical history to share with the provider.
Bring a list of any medications your child is currently taking.
Comment on any changes you have noticed in your child's health since his last checkup.
